Sung Chae, M.D.

Meet Dr. Sung Chae, M.D., the Medical Director of Partnership Health Center South Brunswick (PHCSB). Dr. Chae is Board Certified in both Family Medicine, as well as Lifestyle Medicine, which focuses on a collaborative and team-oriented approach to wellness - something she found very attractive in the PHC model.

Dr. Chae earned her medical degree at the University of Buffalo in Getzville, NY. Her residency was completed at Lancaster General Hospital in Lancaster County PA, one of the most well-respected hospitals in the nation. Her Fellowship in obstetrics was completed at the University of Washington located in Spokane, Washington.

Prior to joining PHCSB, she was a faculty member for over 20 years at JFK University Medical Center in Edison, NJ- now part of the Hackensack/Meridian HealthCare System, where she taught and supervised Family Medicine residents. Although very committed to the medical services offered there, she was looking for a more progressive way to be involved with direct patient care.

PHC offered what she was seeking, namely a coordinated medical model which would be more conducive to handling chronic care when required, in addition to providing many other services not normally found in most medical practices.

Partnership Health Center South Brunswick (PHCSB) offers a variety of on-site medical services for members. Available services include primary and preventative care, urgent care, lab services, X-ray, and professional clinical staff at the Advanced Practice Registered Nursing (APRN) and MD level- all available under one roof and all with no co-pay! Mental health therapy and pharmacy services are also available at your PHCSB.

Dr. Chae believes in a different focus from the traditional disease model, toward one of facilitating wellness through nutrition, sleep, social connectiveness, and physical activity. At PHC South Brunswick, Dr. Chae strives to not only care for members when they are ill, but to assist in achieving overall wellness before chronic disease has set in. As a result, chronic illness can then be reduced or avoided completely when significant changes occur through increased awareness of additional options and approaches.

Joining Partnership Health Centers (PHC) solidifies my goal to practice medicine with all of the collaborations needed to attain patient health and wellness," explained Dr. Melendez Russo, adding, "I very much enjoy the PHC community and helping Franklin Township and Township School members reach the best healthcare outcomes possible."

Dr. Mary McCabe, M.D.

Dr. Mary McCabe, M.D. is the Medical Director of the Toms River Partnership Health Center. She received her training at Stonybrook Medical School in Emergency Medicine and her fellowship was completed at Beth Israel Hospital in NY. Dr. McCabe gained familiarity with New Jersey healthcare services having completed her Masters in Health Care Administration (MHA) at Seton Hall University located in South Orange, NJ.

Dr. McCabe believes that Partnership Health Centers is a very unique company- offering encompassing care to address the fragmentation that many patients experience elsewhere in the healthcare system. Having lab availability, physical therapy, chiropractic care, acupuncture, X-ray, and pharmacy services, as well as mental health therapists, all in one location is unusual but right on point. In Dr. McCabe's view, "it represents the future of medicine."

She tends to take a broader view looking at the interplay of various factors involved in a member's care and believes that good medical care is within each person's control. Her ability to provide preventive care helps to avoid numerous types of problems which might occur later on. Our goal at PHC is to not only treat members when they are ill, but to assist in achieving overall wellness. And, since members can receive wide-ranging care earlier, the Toms River PHC is an important key to achieving good health.

Having worked at the center since 2014, Dr. McCabe regards the team at Toms River PHC as "a truly great one." She believes the staff have done very well over the years in meeting members' goals of better health for the whole person. Dr. McCabe looks forward to continuing to evolve and improve the services offered at Toms River Partnership Health Center.

Dr. Domingo Aviado, M.D.

Dr. Domingo Aviado, M.D., Medical Director of Partnership Health Center Brick, spent many summers on Barnegat Bay, NJ before moving to Brick over 10 years ago. During medical school at Rutgers, Dr. Aviado decided early to focus on primary care medicine and is currently Board Certified in Family Medicine. He is experienced in urgent care, geriatrics, pediatrics, chronic disease management, physicals, and preventive medicine. Dr. Aviado comes from 20 years of experience in private practice in Delaware, known for his emphasis on taking time to listen. It was in California where Dr. Aviado was introduced to the health center model, where all health services are provided in one place, similar to the Partnership Health Center at Brick. This is also similar to the Community Based Outpatient Clinic of the Veterans Affairs in Brick where he was the lead physician.

As Medical Director at Partnership Health Center Brick, Dr. Aviado leads a team of providers with emphasis on quality, comprehensive, accessible care. He believes that in order to achieve better health, one must understand what is important in their lives, and, by focusing on health, all can achieve their goals. Because priorities in life are unique and may change with time, Dr. Aviado looks forward to supporting all that visit the center by helping to identify their issues and improve their wellness.
